Child health records: documents and digital health file
How to organize health documents, vaccine certificates, reports and access to your child's Electronic Health Record.

"Health booklet" or "health record" can mean different things depending on the region and service: paper booklet, certificates, vaccine records, Electronic Health Record, reports and attestations.
The goal is not a perfect binder. It is finding information quickly when needed: pediatric visit, vaccine, emergency care, travel or enrollment in services.
What to collect
- Health card or tax/health code.
- Birth discharge letter.
- Important reports.
- Vaccine certificate or records.
- Pediatrician contacts.
- Allergies, medicines or relevant conditions.
- Travel documents if needed.
- Electronic Health Record access, if available.

Electronic Health Record
The Electronic Health Record is the digital access point for many health documents. Access for minors may require specific procedures, delegations or steps through regional services.
Always verify on the national portal, your Region's website or your local health authority.
Vaccines and visits
Keep:
- vaccine invitations;
- updated certificates;
- any reactions reported;
- next appointments;
- pediatrician notes.
